ICD-10 to HCC Lookup: V24 vs V28

FY2027 update in progress. The FY2027 ICD-10-CM code set (effective 10/01/2026) is still loading, so a code may be missing or a deleted code may still show. Confirm against official CMS files before billing.
Source. CMS "2027 Initial ICD-10-CM Mappings" (codes valid in FY2025 and FY2026) and the CMS 2027 Initial Model Software for category names. CMS-HCC V28 uses 115 payment HCCs; V24 uses 86. The V24 column here is CMS's V24 category from the ESRD/V24 mapping. CMS posts midyear/final mappings later; codes new on October 1, 2026 are not in this file yet.
