What this is. A reference calculation of the raw CMS-HCC V28 score for the six community segments. It does not apply the CMS normalization factor or MA coding-pattern adjustment, and it does not cover institutional, new-enrollee, ESRD or PACE models. Official scores come from CMS. Source: CMS 2027 Initial Model Software (CMS-HCC V28, software V2826.115.T2) and 2027 Initial ICD-10-CM Mappings. These are CMS's initial PY2027 files; CMS publishes midyear/final mappings later.
